GENERAL ATHLETICS INFORMATION

Please note deadlines and provide the required athletic forms to the CHA Health Services Office on a timely basis. As a recognized school of the State of Illinois, and an approved school of the Illinois High School Association (IHSA), all health requirements must be followed.
Required Athletic Forms (Grades 6-12)
Students in grades 6-8 may participate in:
Boys’ & Girls’ Basketball
Thanks to our co-op program, upper school students may participate in most IHSA sports. All students participating in a sport must have a physical exam on file before practice/try-outs begin. Students may use either of these physical exam forms:
All physical exams must be current during a sports season (within 13 months of date of exam). A summer exam is recommended for year-long eligibility. Upper school students should return all required sports physicals and IHSA Consent forms to the CHA Office (who will then forward copies to CLA).

Directions to Away Games for Middle School Opponents
GO BLUE! Come Cheer on our Athletes! CHA and Christian Liberty Academy have formed an IHSA Cooperative Partnership. Mr. Jeff Cook, CHA Athletic Director, is liaison between CHA families and CLA coaches. Practice and game information will be available once teams are formed by the coach.
