All athletes in Grades 6-12 must have the required forms on file with CHA before practices/try-outs begin. Please email the forms as attachments to Health Services with a subject line as follows: Health Forms – Last Name, First Name (for example: Health Forms – Doe, Susie)

As an approved school of the Illinois High School Association (IHSA), which governs the equitable participation in interscholastic athletics and activities, all students participating in athletics must abide by their health requirements. Please be aware that the required health forms for sports participation expires after 395 days. If your forms expire, your child cannot play or practice until new forms are on file. The required forms for student athletes in Grades 6-12 are:

◘ Physical Exam
  • Every athlete must have a current physical (within 13 months of date of exam) during the entire athletic season. For this reason, a summer exam is recommended for year-long eligibility.
  • The Illinois State Physical Exam Certificate or the IHSA Pre-Participation Physical Exam Form will meet this requirement.
◘ IHSA Consent, Asthma, & Concussion Information Form
  • This form is required for all Grades 6-12 student athletes and must be signed by both the student athlete and his/her parent or guardian.
  • This form is due every school year.
